Humana is known for supporting the needs of its clients beyond healthcare. Today, the company announced it would be launching a value-based care payment initiative. This program is being designed to address nonmedical health risk factors affecting Humana members. The program is being coined ‘The Social Determinants of Health Value-Based Program,’ and will try to improve health outcomes using a holistic approach to patient care.
Social Determinants Of Health Value-Based Program
The Humana Social Determinants of Health program aims to bring attention to key factors that pose risks to client health. That includes food insecurity (inconsistent access to nutritious food), social isolation (lack of interaction with other people), loneliness (feeling of being alone), and housing instability (lack of reliable housing). The model will offer tools and resources to help address these social determinants of health. It will also provide compensation to enhance care coordination. The program centers around three components: patient screenings, documentation of screening results, and connecting the patient to appropriate resources.
Statement From Humana VP
A statement was released from Vice President of Population Health Strategy at Humana, Caraline Coats, about the new program. “Humana is committed to advancing the transformation to value-based care, and incorporating a focus on social determinants of health is the logical next step in that evolution.” She went on to state that, “…through this program, Humana is pleased to invest in a new approach that includes focusing on nonclinical barriers to health. We’re better able to support clinicians as they broaden the scope of patient care to improve health outcomes.”
Humana Partnerships
The program has been designed to complement current value-based agreements between Humana and providers. One of its participants is Ochsner Health, Louisiana’s largest non-profit, academic health care system. The company wants to continue moving into the future to offer more to their providers. Recently Humana’s annual Value-based Care Report, issued in November, showed clinicians in value-based arrangements encourage more engagement with Humana MA members compared to providers in non-value based contracts.
