ATTENTION AGENTS! Medico has released revised forms – effective now in all states – for Hospital Indemnity, First Diagnosis Cancer, and Final Expense Whole Life! They are available on the Medico Information Center (MIC) website. This includes applications, application booklets, replacement forms, receipts, outlines, and state specific forms.

Please be advised: Old forms will no longer be accepted after April 30, 2017.

Though a relatively small change, revisions removed references of ‘Corporate Office’ and ‘Administrative Services’ in the heading of the forms. In doing this, the version dates also needed to be updated. Now the forms have been stamped with the date code of 0317 in the bottom right hand corner. Example: 34 112 1005 0317 US.
